Summary
New Jersey established the DDD Supports Program (effective July 1, 2015), a Medicaid waiver demonstration for adults 21+ with intellectual and developmental disabilities. Providers must enroll through a combined Medicaid/DDD application, obtain NPIs for each service location, and submit claims to Molina Medicaid Solutions. This newsletter provides enrollment FAQs and clarifies billing requirements for DDD-funded community-based services.

REQUIREMENTS: By enrollment completion: (1) Billing team and providers must complete the combined Medicaid/DDD provider application through the DDD Provider Portal at http://www.nj.gov/humanservices/ddd/programs/supports_program.html. (2) Obtain a unique National Provider Identifier (NPI) for each service location via NPPES at https://nppes.cms.hms.gov/NPPES/Welcome.do. (3) Ensure each service location is separately enrolled and approved—services rendered at non-approved locations will be denied. (4) For HIPAA 837P claims: report NPI only as billing and rendering provider. For paper/DDE claims: report both Medicaid Provider ID and NPI. (5) All DDD-funded community-based service claims with dates on/after July 1, 2015 must be routed to Molina Medicaid Solutions. (6) Sign required Business Associate Agreement and FD-62 agreement with DMAHS. (7) Direct enrollment questions to DDD Support Program Help Desk (DDD.SuppProgHelpDesk@dhs.state.nj.us) and claims submission questions to Molina at 1-800-776-6334. Failure to comply will result in claim denials for services rendered at unapproved locations and processing delays.
