Immediately: Providers submitting genetic assay testing claims to NJ Medicaid must either (1) use the revised FD-431 form and report 'Date Ordered' instead of 'Date of Service', or (2) attach existing test requisition forms that include all required elements: provider info with Medicaid ID, member info with Medicaid ID number, gene assay testing details with date ordered and ICD-10 codes, reason for testing, description of future medical management impact, and authorized signature. Billing team should update workflow to ensure proper form completion and attachment to all genetic testing claims.
