Summary
New Jersey is launching a Housing Supports Program effective July 1, 2025, for Medicaid/NJ FamilyCare MCO members. Providers must enroll as Housing Supports Group Providers through Gainwell Technologies by submitting required applications and documentation. This is an MCO-only service (not available under fee-for-service Medicaid) with four specialty types: Pre-tenancy Services, Tenancy Sustaining Services, Residential Modifications and Remediation, and Move-in Supports.

By May 1, 2025: Providers interested in offering Housing Supports services must submit a Housing Supports Group Provider Application to Gainwell Technologies (State Fiscal Agent). ACTION STEPS: (1) Obtain a Type 2 Organization NPI if not already an enrolled Medicaid/NJFC provider; (2) Access the application at www.NJMMIS.com beginning April 15, 2025; (3) Select one or more provider specialties to offer; (4) Gather and submit required documentation including FD-20, FD-62, FD-435, FD-452, Disclosure of Ownership Form, IRS letter (147C or CP-575), NJ Business Registration, and NJDCA Home Improvement Registration (if applicable); (5) Contract separately with each MCO to provide services to their members; (6) Complete credentialing requirements with each MCO network. By July 1, 2025: Enrollment must be complete for providers to bill for Housing Supports services. Note: Housing Supports services are NOT covered under Medicaid fee-for-service; they are MCO-only.
