Summary
This is a reminder that Independent Living Waiver (ILW) providers must enroll as MO HealthNet Consumer-Directed Services (CDS) providers and complete an FMS addendum to be reimbursed for Financial Management Services. Claims are being denied because providers lack the required FMS addendum on file.
Action Required
Immediately: ILW providers billing for Financial Management Services must complete MO HealthNet FMS addendum if not already on file. Contact Provider Enrollment at mmac.ihsconracts@dss.mo.gov or download form from MMAC website. Billing team must ensure at least one unit of T1019 U6 is delivered before billing T2040 U6 each month. Verify all State Plan CDS Personal Care (T1019 U2) is exhausted before billing CDS Personal Care - ILW (T1019 U6). Claims will be denied without proper enrollment and addendum.
