Summary
CareSource Indiana Medicaid is requiring providers to use the updated HHS-687 Consent for Sterilization form (CMS version) for all sterilization procedure consents signed on or after October 10, 2025. The expired version with July 31, 2025 expiration date is no longer accepted after October 9, 2025, but previously signed forms do not require reconsent.

By October 9, 2025: Obtain and implement the updated HHS-687 Consent for Sterilization form from CMS for all sterilization procedures. Providers and clinical staff must ensure the new form is used for all patient consents dated on or after October 10, 2025. Update patient intake and consent processes to remove the old HHS-687 version. No reconsent is required for patients who previously signed the expired version. Failure to use the updated form for signatures on or after October 10, 2025 may result in claim denials or compliance issues with CareSource Indiana Medicaid.
