Summary
Missouri Medicaid providers who lose or destroy medical records due to natural or man-made disasters must complete an attestation form within 30 days of the event. This new policy provides a formal process for documenting medical record loss to MMAC.
Action Required
Within 30 days of any disaster causing medical record loss: Complete the Attestation of Medical Record Loss or Destruction form on the MMAC website. Contact MMAC.Attestations@dss.mo.gov or 573-751-3399 with questions. Billing team should save this process for disaster preparedness procedures.
