Billing team must ensure all requests include facility NPI and Community Psychiatric taxonomy code, use updated Prior Authorization Request form indicating initial/re-authorization in box 1, include dates (fields 19-20) and units/days (field 22), and fax to (573) 659-0207. Clinical staff must provide treatment plans and prescriber progress notes with all requests. In eMOMED billing system, select Community Psychiatric taxonomy code when billing H0019, H2020, H2022 after entering facility NPI.
