Summary
MO HealthNet Division has moved all provider forms to a new Provider Forms page organized by commonly used forms, Exception Requests, and Pharmacy Prior Authorization forms. Several forms have been updated to be more consistent, electronically fillable, and easier to complete.
Action Required
Immediately: Update bookmarks and internal documentation to reference the new Provider Forms page at mydss.mo.gov. Train staff on the new form organization structure with sections for commonly used forms, Exception Requests, and Pharmacy Prior Authorization forms. Contact MHD.Education@dss.mo.gov if unable to locate any needed forms.
