Missouri HealthNet has launched new simplified online forms for providers and participants to report incurred charges for spend down verification. The new online MO HealthNet Spend Down Provider form replaces the previous paper form (MO 866-4501) and allows providers to verify qualified expenses when actual bills aren't yet available.
Action Required
Immediately: Missouri providers serving MO HealthNet patients with spend down requirements must transition to using the new online MO HealthNet Spend Down Provider form (IM-29PROV) accessible at https://mydssupload.mo.gov/UploadPortal. Stop using the old paper form MO 866-4501. Train billing staff on completing the online form to verify patient expenses when actual bills are unavailable. Ensure staff understand they can also fax completed forms to 855-600-3754 if needed.
