Summary
The Division of Senior and Disability Services transitioned from CyberAccess to a new electronic case management system called HCBS Fusion on May 5, 2025, for Home and Community-Based Services. Providers need to familiarize themselves with the new system for submitting referrals, reviewing care plans, and completing reassessments.

Immediately: HCBS providers must transition to using HCBS Fusion system for all case management activities including submitting referrals, reviewing care plans, and provider reassessments.
