Summary

Effective January 1, 2026, MDwise will no longer serve as a managed care plan option for Indiana Medicaid members (HIP and Hoosier Healthwise). Members must select a new plan or be auto-assigned. CareSource is actively recruiting these members, but Walgreens and Kroger pharmacies are NOT in CareSource's network, requiring practices to transition patient prescriptions to in-network pharmacies before the deadline.

By December 31, 2025: Billing and clinical teams must identify all active MDwise patients currently using Walgreens, Kroger, or other out-of-network pharmacies. Contact these patients and providers to transition prescriptions to CareSource in-network pharmacies effective January 1, 2026. Use CareSource's Find a Pharmacy tool to locate alternatives. Send new prescriptions to in-network pharmacies for use on or after January 1st. Ensure all prescriptions filled on or before December 31, 2025 route through MDwise network to avoid coverage gaps. Update patient records and pharmacy contact lists in billing system. For questions, contact CareSource Provider Services at 1-844-607-2831 (Monday-Friday, 8 a.m.-8 p.m. ET). Failure to transition prescriptions may result in patient out-of-pocket costs or coverage denials.
