Summary
CareSource Indiana Medicaid now covers Fluoride Varnish application in medical offices (ages 0-4) using CPT 99188, in addition to existing dental coverage (D1206). Medical providers (physicians, PAs, APRNs) must complete certified training before billing for this service. This expands access to preventive oral health services for Hoosier Healthwise and HIP plan members.
Action Required
By February 28, 2025: (1) Billing team must verify that all eligible medical providers (physicians, PAs, APRNs) have completed certified training before allowing CPT 99188 to be billed; maintain documentation of training completion in credentialing files. (2) Update billing system to recognize CPT 99188 for ages 0-4 under Hoosier Healthwise and HIP plans only. (3) Add CPT 99188 to claim scrubbing rules to ensure claims include age verification (0-4 years) and documentation of provider supervision requirement. (4) If practice provides medical Fluoride Varnish services, providers must complete required certified training before rendering service. Claims submitted without documented provider training or for patients outside age 0-4 will be denied. (5) Clinical staff should refer patients to dental home for comprehensive evaluation post-application per policy requirements.
