Summary
CMS requires all Medicare Special Needs Plan (SNP) providers to complete mandatory annual Model of Care (MOC) training for UPMC for Life Complete Care members. Providers must complete online training at upmchp.us/snpmoc using their tax ID number by February 29, 2024.
Action Required
By Feb 29, 2024: All providers treating UPMC for Life Complete Care (HMO-SNP) members must complete mandatory MOC training at upmchp.us/snpmoc. Use tax identification number to sign in and attest to completion. Contact Provider Services at 1-866-918-1595 with questions. Failure to complete training may affect provider participation in SNP network.
