Summary
Hospira has issued a voluntary nationwide recall of Sodium Bicarbonate Injection (4.2% and 8.4%) and Atropine Sulfate Injection due to glass particles found during routine inspection. Healthcare providers must immediately stop using affected lots and return or discard them to prevent potential adverse events including local irritation, blood vessel blockage, or life-threatening blood clots.
Action Required
Immediately: Clinical staff must identify and remove all affected lots of Hospira Sodium Bicarbonate Injection (4.2% and 8.4%) and Atropine Sulfate Injection from inventory. Check all medication storage areas and emergency crash carts. Return affected products to supplier or dispose of properly. Contact Sedgwick Inc. at 1-800-805-3093 for assistance. Notify providers to use alternative products for patients requiring these medications.
