UPMC Health Plan permanently closed their HIPAA fax line (412-454-7829) effective February 1, 2025. Personal Representative Designation (PRD) forms must now be submitted via the member's digital account online or mailed to a specific PO Box address.

Immediately: Stop using UPMC Health Plan HIPAA fax line 412-454-7829 for PRD forms. Direct members to submit PRD forms via their Member Site digital account for fastest processing. For paper submissions, mail to: UPMC Health Plan ATTN: Medicare Enrollment PO Box 2965 Pittsburgh PA 15230-2965. Always include member's name, DOB, and Member ID on each page when mailing HIPAA documents.
