Medical Mutual is implementing major changes to prior authorization processes effective January 1, 2021. Outpatient therapy and chiropractic services will now require prior authorization directly through eviCore healthcare instead of Landmark Connect, and providers can no longer submit requests via the old system.

By January 1, 2021: Stop submitting prior authorization requests via Landmark Connect. Set up accounts with eviCore healthcare at www.evicore.com/pages/ProviderLogin for web submissions, or use phone (877-531-9139) or fax (855-774-1319) options. Attend eviCore orientation training at https://www.evicore.com/resources/healthplan/medical-mutual-of-ohio. Update workflow to obtain prior authorization from eviCore before providing outpatient therapy and chiropractic services - services without prior auth may be denied and cannot be billed to patients.
