By May 15, 2026: Hospital billing and discharge teams must transition to the new CMS-10065 Important Message from Medicare form and updated Detailed Notice of Discharge form. Download new English and Spanish versions from CMS's BNI webpage. Update discharge processes to use new forms for all Medicare beneficiaries (Original Medicare and Medicare Advantage). Old forms may be used until the deadline but must be replaced by May 15, 2026.
