Summary
Healthfirst has updated their Special Needs Plan Model of Care training for 2024, shifting Individualized Care Plan (ICP) reports from postal mail to the provider portal only. Providers must now access ICP reports online and provide feedback within 14 days, or Healthfirst will assume agreement with the care plan.
Action Required
Within 14 days of receiving ICP notifications: 1) Billing team and providers must access HFproviderportal.org to retrieve ICP reports under Provider Letters > Care Plans (no longer mailed). 2) Providers must review ICP reports, contact Healthfirst at 1-888-801-1660 with any changes, and keep copies in patient charts. 3) Front desk staff should create portal accounts if not already established. 4) Providers must discuss ICP reports with SNP patients at next appointments. Failure to respond within 14 days means automatic agreement with care plans.
