By May 22, 2026: Each prescriber who prescribed controlled substances to Oklahoma Medicaid patients between Oct 1, 2024-Sept 30, 2025 must complete the anonymous PDMP survey. Complete online at https://forms.cloud.microsoft/g/C1m01s9Qa2, email to pharmacy@okhca.org, mail to OHCA Attn: Pharmacy Department PDMP Survey, or fax to 405-530-7137. Each prescriber must individually complete their own survey.
