When submitting MCO inquiries about missing WRAP payments, include the Supplemental Claims Inquiry Spreadsheet with all required data elements from Table 1. Contact appropriate MCO using provided email addresses with subject line 'FQHC/RHC Action Required'. Failure to reconcile within 90 days may result in incomplete threshold encounter data in KYHealthNet.
