Each Category II code must include specific date of service for individual prenatal and postpartum visits. Update billing software to handle multiple line items with no-charge Category II codes. Train providers on documentation requirements. This applies to Fee-for-Service providers (mandatory) and is recommended for FQHC/RHC providers.
