Aetna updated its vitiligo treatment policy (CPB 0422) to establish medical necessity criteria for photochemotherapy, excimer laser, and narrow-band UVB therapies, requiring documented inadequate response to topical tacrolimus, corticosteroids, or ruxolitinib before approval. The policy designates numerous treatments as experimental/investigational (including JAK inhibitors combined with UVB, CAR-T therapy, rituximab, and various grafting techniques) and classifies micropigmentation and depigmentation as cosmetic, not covered.
